TARGET CITIES PROGRAM DESCRIPTION

There are three community based, and adolescent points of entry into the Target Cities system of services. This gives the client and family accessibility to the Alcohol and Drug Addiction Services Board funded agencies and other identified services, which form a dynamic and developing support Systems network There are currently four other points of entry into the Cleveland C.A.R.E.S. network which address the specialized needs of clients identified through criminal justice arenas.

It is the goal of Cleveland C.A.R.E.S. to reduce treatment barriers in the City of Cleveland through comprehensive needs assessment. service brokerage and linkages of an array of social services. These services may include, but are not limited to: residential or outpatient counseling/therapy, transportation, child care, primary health care, intervention, crises stabilization, spiritual outreach, job training, and job placement.

Finally, the Target Cities Program works aggressively to monitor the progress of clients through process, outcome and impact evaluation efforts. It is critical to meet the ongoing challenges of collaboration, coordination, client accessibility and communication needed to appropriately document Target Cities progress per established goals and objectives.

CLEVELAND C.A.R.E.S. CRIMINAL JUSTICE COMPONENT

The Criminal Justice Component of the Cleveland C.A.R.E.S. Target Cities Program is one that continues to develop to meet the comprehensive reels of chemically dependent offenders. This component consists of four major areas that target particular offender populations.

The Treatment Alternatives to Street Crime (T.A.S.C.) Program is the identified Central Intake Unit for clients involved with the Criminal Justice system. T.A.S.C. is the point of entry into the Cleveland C.A.R.E.S. Program which provides specialized services to criminal justice referrals. Clients receive a range of assessment, treatment referral, case management, and drug screen services. Maria Nemec is the Program Manager. She can be reached at 443-8250. Clients are referred through the Cuyahoga County Court of Common Pleas, the Cuyahoga County Probation Department, the Cleveland Municipal Probation Department and the Adult Parole Authority.

Another facet of the Cleveland C.A.R.E.S. Criminal Justice component is the expansion of the Center Point Project into the Cleveland House of Correction. Center Point is an intensive outpatient program that is under the direction of the Office of Mental Health and Substance Abuse at the City of Cleveland Public Health Department. The program provides pre-treatment, treatment and continuing care for those identified Target Cities clients incarcerated in the House of Correction. Clients will he identified through an ongoing educational process and assessment. Clients needing and requesting substance abuse counseling will receive services On Site and will continue in the outpatient Center Point Sites.

An integral part of the Criminal Justice component is the Community Assessment Foundation intensive day treatment program, with residential services, located at St. Alexis. Hospital Medical Center, 5163 Broadway, Cleveland, Ohio. This 16 bed program provides individual and group counseling for males and females, ages 18-35. who are referred by criminal justice Systems. It is supported arid partly funded by the Ohio Department of Rehabilitation & Correction.

The County Jail provides Target Cities services to those individuals identified, via pretreatment services, as needing help. Alcoholism Services of Cleveland provides pretreatment, assessment and treatment services for chemically dependent County jail incarcerated Cleveland residents. The Daywood II Intensive Day Treatment Program is the primary treatment program for clients who have received Target Cities services during incarceration. This program, as well as those discussed above, are linked with the Central Intake Units to insure coordination, comprehensive assessment and case management services in a holistic approach to treating the offender population.

CLEVELAND C.A.R.E.S. HEAD START INITIATIVE

The Cleveland C.A.R.E.S. Head Start Initiative is a collaborative effort in the Cleveland area to increases the enrollment of children from substance abusing families who are in treatment into Head Start provide them with in enriched educational environment and decrease the cycle of abuse by their families. Additionally, this initiative is implemented to increase the enrollment of substance abusing Head Start parents into an enhanced treatment environment offered by the Alcohol and Drug Addiction Services Board of Cuyahoga County through the Cleveland C.A.R.E.S. Program.

This is a pilot initiative which will focus on six east side Head Start centers and six west side centers by a phase-in approach. Drug and alcohol screening will be an official part of the overall assessment and application to Head Start. For parents identified as in need of chemical dependency services, via the screening process, referrals will be made to Cleveland C.A.R.E.S. for a comprehensive assessment and coordinated service brokerage.

There is ongoing training and cross training to enhance sensitivity levels and continuity of care. The training targets child care workers, teachers and administrators in the designated Head Start centers. as well as the Cleveland C.A.R.E.S. staff. The training helps to establish referral networks and protocols that insure effective services and smooth client-utilization and movement through support systems.

It is expected that both the treatment and Head Start experiences may interact beneficially to encourage the parent to participate more fully in treatment. In a similar way, it is expected that participation in treatment will impact the child's participation in Head Start in an advantageous way. Measures studied are tardiness and absenteeism records.

A Family Intervention Specialist is assigned to each Central Intake Unit to coordinate referrals and services for participants in this initiative. The person assists with referrals to child care, educational services, health related services not available through Head Start, and/or for children not eligible for Head Start. Finally. the Family Intervention Specialist serves as a liaison and resource to the Cleveland C.A.R.E.S. staff, helps to provide a supervised, safe and nurturing environment for children at the Central Intake Unit while parents are under going the assessment process, and assists with training, orientation and community education.

CLEVELAND C.A.R.E.S. FAITH INITIATIVE COMPONENT

The Faith Initiative component of the Cleveland C.A.R.E.S. Target Cities Program is an exciting one. It is our effort to mobilize religious leaders across religious affiliations and geographical locations, to work with the clients and family members. Understanding the role of spirituality in recovery is critical to recovery success. We believe that spiritual leaders and their assistants have much to share with treatment and prevention providers regarding how to keep people motivated and inspired concurrent with a broadened sensitivity to one's religious status.

On the other hand, we are challenged by the need to provide clergy with more information and tools on how to identify, respond and refer members suffering from substance abuse. Therefore, one of the roles of the Faith Initiative is to set up cross training between systems. We have sponsored two training events, including a seminar and a conference. In addition members participate in seminars sponsored by the Alcohol and Drug Addiction Services Board of Cuyahoga County.

The Faith Initiative members are encouraged to meet with inspire, and pray for the clients participating in the program. Members may visit clients at the Pro-Treatment Unit to offer spiritual counseling. We are commissioned to facilitate the development of more church based after care and support groups, with some faith organizations forming community clusters to share the responsibilities of hosting. Religious leaders provide inspirational messages to clients and offer support, via a special telephone system called the TelePratice Connect System, available through Cleveland C.A.R.E.S.

The Faith Initiative is a national movement of Target Cities, conceived by Dr. Beny Primm of the Center for Substance Abuse Treatment. Cleveland is one of the few Target Cities that has an active faith initiative effort. This group meets on a regular basis and welcomes your participation and recommendations of other committed leaders in the faith community. Reverend Richard McCain is the Convener of the Faith Initiative. The staff liaison is the Program Operations Manager. CLEVELAND C.A.R.E.S. CARE MANAGEMENT COMPONENT

The Care Management Component of 'he Cleveland C.A.R.E.S. Target Cities Program is a critical one. The program rests heavily upon how well the care managers are able to advocate, negotiate, and access services for the clients and their family members. The care managers build resources on a daily basis to meet the comprehensive needs of City of Cleveland residents Liking advantage of Target Cities.

Care Managers are commissioned to establish a CARE PLAN with the clients, which outlines the services and programs necessary to meet the identified needs of the client. The plan is developed with the client arid the other clinical professionals on the unit. The care manager provides service brokerage to connect the client with service and support systems that will help them reach their treatment goals and objectives.

The Care Manager's role is a 'racking and monitoring one. The client's progress will be followed, supported and monitored from six months to a year after their admission to Target Cities. The Care Managers attend treatment case conferences, and serve as part of the support system for the treatment facility and the client. This is partially made possible by a special treatment enhancement component known as the TelePractice Connect System. TelePractice is a unique telephone system that clients and service providers have at their disposal. TelePractice allows for care managers to communicate to their clients, via a telephone message bank system, things such as: reminders for appointments, available services, motivational/inspirational messages, etc. They can also facilitate health related topic discussions, via 'he phone, giving participants the ability to add comments and answer questions without leaving their premises. Among many other benefits, it also allows service providers to communicate to each other regarding client needs and concerns.

The Care Managers have come to the program with years of experience in the field of chemical dependency. They participate in ongoing training and visit referral and treatment Sites. The Care Managers are available for consultation throughout the system of treatment and prevention agencies, and welcome your feedback and support.

CLEVELAND C.A.R.E.S. ADOLESCENT SERVICES COMPONENT

The Cleveland C.A.R.E.S. Program has developed a specialized response meet the needs of the adolescent population involved with the criminal justice system, or at risk for involvement in the criminal justice system.

The implementation of coordinated and enhanced central intake arid assessment services for adolescents is accomplished through a contractual agreement and accompanying collaborative service and linkage arrangements with the CIU. The Youth Chemical Dependency Unit of the Catholic Social Services (CSS) will serve Cleveland C.A.R.E.S. eligible adolescents including juvenile offenders in coordination with the CIU. Youth who are Medicaid eligible will have access to treatment resources through CSS Medicaid Adolescent Rehabilitation Program (MARP). City of Cleveland residents who did not have Medicaid also receive comprehensive substance abuse assessment services primary health and mental health screening, and linkages to adolescent treatment programs in the community. Over 200 additional adolescent residents of the City of Cleveland are expected to be served through our contract with Catholic Social Services. Their staff are a part of a community wide planning and development team of organizations and Systems addressing substance abuse issues of assessment and case management for adolescents.

Assessments for adolescents (ages 13-18 years old) including those with Medicaid coverage, will be provided On site at the Cleveland C.A.R.E.S. Central Intake Units and at Catholic Social Services which is located at 3135 Euclid Avenue Suite 202. The phone number for Catholic Social Services is 391-2040. Alisa Bridges is the intake coordinator. Maureen Dee is the adolescent chemical dependency program administrator. Angela Harvey is the adolescent chemical dependency assessment counselor and case manager assigned to serve Cleveland C.A.R.E.S. eligible youth and families. Family interventions are scheduled as needed at all the Sites. This is a method that attempts to coordinate the efforts of significant people impacting the adolescent's life. to help identify any chemical dependency related symptoms that have been observed, and to provide encouragement to the adolescent to accept any treatment recommended.

A multi-disciplinary team works together to develop a Care Plan for the adolescent and family, to help insure holistic services Referrals and linkages are made to prevention arid support groups as needed per the Care Plan. The Youth Chemical Dependency Services Unit provides ongoing personal links, monitoring and tracking for the adolescents and their family members as they progress along the Care Plan continuum for an average of 12 months for adolescents completing treatment, including aftercare. A significant proportion of the Target Cities expansion funds were awarded to two adolescent providers: the Covenant (574-9000) and Parmadale Outpatient Chemical Dependency Program (845-7700) to host arid operate the Continuing Care (After Care) Services to adolescents and their family members

Overall services will be evaluated for treatment effectiveness and efficiency by the length and quality of the adolescents recovery
